After sitting on the phone on hold for 5 minutes – someone came on line and brought up my name and address but I had not been there…. She said let me find a chart – then she said hold on a minute and another woman came on the phone – she said oh you need to call the billing office – I said but I have never been there I should have no bills. She said they have an office on Ormond Beach with Dr. Malik.

I said I had been to see him ONCE way back in 2001 AND that had been settled and the lawsuit had been settled. I explained the forms came from HER office with HER office address – no phone numbers on any of the papers and how I had to go online to find the phone number. She said billing office sent it out and got my name mixed up with someone else’s. She said to disregard the item. So I am.
