After the Titanic sank, there was a great deal of talk about revising maritime legislation and making boats and ships a great deal safer.

After a plane crash, there is an immediate review of what went wrong, and how it can be avoided in the future.

From what I understand – if someone dies during a hospital procedure, there is a review  by everyone involved to find out what went wrong, why the person died and how it can be avoided in the future.

When there is a pile up, a terrorist attack, an outbreak of disease – anything that a) leaves people dead and b) possibly could have been avoided – there is a review by the groups involved (the groups who were responsible for ensuring this sort of thing doesn’t happen) to see what mistakes were made, how the event came to pass and how they can prevent something similar coming to pass again in the future.

It makes perfect sense if you ask me – the time to talk about preventing deaths is just after the deaths have happened, because people will be focused on it and thinking about it.
