Waiting for 9am

I got a very weird thing in the mail yesterday.

It was an insurance medical form from some pain management office in Orange City FL.

The note with it said dear (not my first name but my middle name and my last name) Enclosed is a form from the office of insurance regulation. This form must be signed in order for your auto insurance company to pay for claims. Please sign and return in enclosed envelope. Thank you.

Now here is the funky part – I have never been to a pain management office in Orange City. I see a doctor here in Daytona. AND my accident was in 2000 and I have already settled the lawsuit. So WHAT IN THE HECK IS GOING ON?

The note did not have a phone number or even a signature but it did have the address on both envelopes so I went to realpages.com and looked them up. I have the phone number and called yesterday after 5pm got the service who told me they open at 9 today so I am calling them in exactly 3 minutes.

I have no idea what is going on but I AM going to get to the bottom of it.

I wouldn’t trust it, it could be someone trying to forge your signature. Just throw it away. If they have a real problem direct them to your lawyer. You shouldn’t hear from them again. Or you could contact the lawyer and ask why they are asking for this information four years later!
